Physiology Interview Questions in Medicine and Vet Interviews

The renal problem Oxford publishes with its own marking notes, and why the UCAT deadline lands twelve days before the ESAT

Physiology is not a degree you apply for at Oxford or Cambridge, but it sits behind at least four different applications there. Oxford’s own published sample question for Biomedical Sciences — why sugar in the urine indicates diabetes — is a renal physiology problem, answerable entirely from A-level biology and chemistry. Medicine applicants meet the same reasoning behind a different test: the UCAT, whose booking window closes on 16 September 2026, twelve days before the ESAT’s.

Which Oxbridge Courses Examine Physiology at Interview?

The confusion behind most searches for physiology interview questions is that there is no Physiology course to apply to. What there is instead is a body of material — how organs work, how transport and regulation happen, why a system fails — that turns up at interview for several different degrees, behind several different admissions tests.

The pattern in that table is worth naming. Physiology is examined most directly where it is not the named subject: Oxford’s Biomedical Sciences sample question is renal physiology, while its Medicine sample question is epidemiology. A candidate who prepares physiology because they are applying for Medicine, and skips it because they are applying for Biomedical Sciences, has it exactly backwards.

The entry requirements sit close together and differ in ways that matter. Oxford Medicine asks for A*AA with Chemistry and at least one of Biology, Physics, Mathematics or Further Mathematics. Oxford Biomedical Sciences asks for A*AA with two A-levels from Biology, Chemistry, Physics and Mathematics, and notes that there are no formal GCSE requirements for the course. Cambridge Veterinary Medicine asks for A*AA with Chemistry and at least one of Biology, Mathematics or Physics — and adds two conditions found on almost no other course page: you cannot apply if you will not be 18 by 1 March of your first year, for safety and insurance reasons, or if you have failed or been excluded from another veterinary school.

The Physiology Question Oxford Publishes, Worked Through

Oxford publishes a Biomedical Sciences sample question on its sample interview questions page, from Robert Wilkins, of St Edmund Hall: “Why is sugar in your urine a good indicator that you might have diabetes?” It is unusually well documented, because Oxford also publishes what a good answer does with it.

Oxford’s framing first: the question “builds on general knowledge and material studied at school in biology and chemistry to assess how students approach a clinically-relevant problem. It’s commonly known that diabetes is associated with sugar (glucose) in the urine; this question asks students to think about why this occurs.” So the observation is handed to you. The explanation is the work.

The chain has three links, and Oxford names them all. The kidneys filter blood to remove waste such as urea, but useful substances including glucose are filtered too and must not be lost. Glucose is not normally in urine, so something recovers it: “reabsorption by a carrier protein that binds the glucose molecules and moves them out of the renal tubule and back into the blood.” And then the step that separates a good answer from a complete one — the carrier behaves like an enzyme, so “the capacity to reabsorb glucose is finite because once all of the carriers are working maximally, no further glucose reabsorption can occur.”

Oxford states the conclusion a successful applicant reaches: “an elevated level of glucose in the blood in diabetes leads to increased filtration of glucose by the kidneys and saturation of the carriers that perform the reabsorption, resulting in ‘overspill’ of glucose in the urine.”

Notice what is not in that answer. No renal threshold value, no transport maximum in millimoles, no clinical guideline. Every element — filtration, carrier proteins, enzyme saturation — is A-level material. The difficulty is that nobody has ever asked you to put those three in that order, and that is the whole point of the question.

Why Physiology Questions Take That Shape

A physiology question at interview has a job that a written test cannot do. The test has already measured whether a candidate can solve problems quickly and accurately; the interview is measuring whether they can build an explanation for something they have not been taught, and revise it when someone pushes.

That is why the questions are so consistently of the form “here is an observation, account for it”. Why sugar appears in urine. Why a freshly calved cow runs short of calcium, the question our Cambridge vet med page works through. Why some habitats support more species than others. In each case the observation is common knowledge and the mechanism is not, so the candidate has to reason from what they know rather than retrieve what they have learned.

It is also why Oxford’s Medicine question is not physiology at all. Andrew King of Exeter College asks candidates to put Bangladesh, Japan, South Africa and the UK in order of crude mortality — a question Oxford says “invites candidates to think about a public health question and epidemiology that can be approached in many different ways, without necessarily knowing anything about specific mortality rates around the world.” Same structure, different content. Oxford describes Medicine interviews as gauging “candidates’ understanding of the science underpinning the study of medicine, as well as skills in scientific enquiry”, and physiology is one route into that rather than the whole of it.

Two Booking Deadlines, Twelve Days Apart

Before any of that matters, there is an administrative problem, and unlike a weak answer it cannot be recovered from. Medicine and the science courses use different tests with different timetables, and the Medicine one closes first.

An applicant holding a Medicine choice and a Biomedical Sciences or Natural Sciences choice has two separate registrations, two separate booking systems and two deadlines twelve days apart. The UCAT window also differs in kind: it is a long test window running from 13 July to 24 September 2026, so a candidate chooses a date within it, whereas the ESAT is a fixed sitting on 12–16 October 2026. Missing the UCAT booking deadline of 16 September at 15:00 does not leave an October fallback.

Biomedical Sciences shortlists the smallest share of its applicants of the three and makes offers to the smallest share, from much the smallest intake. Medicine interviews a larger proportion than Biomedical Sciences and converts a larger proportion of those interviewed. Biochemistry interviews the largest share of all, which is consistent with its having no test and no written work to filter on beforehand — the sorting has to happen somewhere, and for that course it happens in December.

None of that ranks the courses by difficulty, and it would be dishonest to present it that way: the applicant pools are different and self-selecting. What it does tell a candidate is where their application is most likely to be decided, and therefore what to spend October and November on. For a Biomedical Sciences applicant, most of the attrition happens before anyone speaks to them. For a Biochemistry applicant, most of it is not. How to Prepare for Physiology Questions

Take Oxford’s own account of the glucose question as the specification, because it is one. Three things follow from it, and none of them is reading ahead into undergraduate physiology.

Work from your syllabus, not past it. Filtration, carrier proteins, enzyme kinetics, feedback regulation, gas exchange, osmoregulation: the raw material of every published question of this kind is A-level content. A candidate who has read a first-year textbook and cannot assemble three familiar ideas under pressure is worse prepared than one who knows the syllabus cold and has practised using it.

Say the middle of the reasoning out loud. Oxford describes prompting a candidate who stalls, and a prompt can only be aimed at reasoning the interviewer can hear. Answers that arrive whole and unexplained are harder to credit than answers that are visibly under construction.

Practise the second question, not just the first. Published questions almost always have an extension, and it is normally where the discrimination happens. In the glucose question, the first part gets you to reabsorption; the extension is the finite capacity of the carrier and the overspill that follows.

What kind of physiology questions come up in an Oxbridge interview?

Questions that start from an observation you could have made yourself and ask you to explain it with A-level biology and chemistry. Oxford’s own published Biomedical Sciences example is exactly this shape: “Why is sugar in your urine a good indicator that you might have diabetes?” Nothing in it requires knowledge beyond school, and Oxford says the question “builds on general knowledge and material studied at school in biology and chemistry to assess how students approach a clinically-relevant problem.”

Do I need to revise physiology beyond the A-level syllabus?

No, and the published question shows why. Its full solution uses filtration in the kidney, reabsorption by a carrier protein, and the fact that a carrier saturates in the same way an enzyme does — all of it A-level content. What makes it hard is that you have never been asked to assemble those three ideas in that order. Reading further into undergraduate physiology is not what closes that gap; practising the assembly out loud is.

What is the model answer to the glucose in urine question?

Oxford sets it out itself. The kidneys filter glucose along with waste products, and it is normally all recovered by a carrier protein that moves it out of the renal tubule and back into the blood. That carrier behaves like an enzyme, so its capacity is finite: “once all of the carriers are working maximally, no further glucose reabsorption can occur.” In diabetes the raised blood glucose means more is filtered, the carriers saturate, and the excess overspills into the urine.

Is the UCAT deadline the same as the ESAT deadline?

No, and this catches people out every year. UCAT booking for 2027 entry runs from 23 June to 16 September 2026 at 15:00 UK time. ESAT booking runs from 20 July to 28 September 2026 at 6pm UK time. That is twelve days between them, and the earlier one belongs to Medicine. An applicant holding a Medicine choice and a science choice has two separate registrations and two separate deadlines, and the UCAT one arrives first.

Are Medicine interview questions all physiology?

No. Oxford’s published Medicine sample question is an epidemiology problem — ordering Bangladesh, Japan, South Africa and the UK by crude mortality — and Oxford describes Medicine interviews as gauging “candidates’ understanding of the science underpinning the study of medicine, as well as skills in scientific enquiry.” Physiology is one of the routes into that, alongside epidemiology, ethics and data interpretation. Preparing only physiology is preparing for part of the interview.

Which is harder to get into, Medicine or Biomedical Sciences?

On Oxford’s published three-year averages for 2023–25, Biomedical Sciences shortlists a smaller share of applicants than Medicine does: 24% interviewed against 29%, and 8% successful against 11%, from an intake of 45 against 155. Neither figure makes one course a soft option. What they do show is that a smaller course does not mean an easier one, and that the Biomedical Sciences filter falls harder before the interview than after it.

When are Medicine and Vet interviews held for 2027 entry?

Oxford states that shortlisted candidates are invited to at least one online interview, expected to take place in December 2026. Cambridge’s main interview period runs from 7 December to 18 December 2026, with most invitations sent in November, plus a winter pool round in mid to late January 2027. Cambridge adds that it cannot reschedule interviews except in exceptional circumstances, so that fortnight has to be kept clear.
